Nalidixic Acid Chin Tien

Nalidixic acid is a synthetic narrow spectrum antibacterial. It is bacteriostatic or bactericidal depending on the concentration. Nalidixic acid appears to act by inhibiting bacterial DNA synthesis, possibly by interfering with DNA polymerization. It is rapidly and completely absorbed from the G.I. Tract. Parent drug and active metabolites are distributed to most tissues specially to the kidney and to the urine.

জটিল ও দীর্ঘমেয়াদী মূত্রনালীর সংক্রমণ। সালমােনেলা ও শিগেলা সংঘটিত মূত্রনালীর সংক্রমণেও ইহা নির্দেশিত। ন্যালিগ্রাম (Nalidixic Acid Chin Tien) চাইল্ডহুড শিগেলােসিসে এম্পিসিলিনের বিকল্প হিসেবে ব্যবহার করা যায়। মাল্টিড্রাগ রেজিস্ট্যান্ট শিগেলা দ্বারা সংঘটিত দীর্ঘমেয়াদী সংক্রমণে ন্যালিগ্রাম (Nalidixic Acid Chin Tien) ব্যবহার করা যায়।

Nalidixic Acid Chin Tien খাওয়ার নিয়ম / ব্যবহারের নিয়ম

• প্রাপ্তবয়স্ক : ১ গ্রাম (২টি ট্যাবলেট) ৬ ঘন্টা পর পর মােট ৭ দিন।
• দীর্ঘমেয়াদী চিকিৎসায় ৫০০ মি.গ্রা, প্রতি ৬ ঘন্টা পর পর।
• শিশু : ৩ মাসের অধিক বয়স্ক শিশুদের ক্ষেত্রে প্রারম্ভিক মাত্রা দৈনিক ৫৫ মি.গ্রা. / কেজি, (দৈহিক ওজন) ৪টি বিভক্ত মাত্রায়।
• দীর্ঘমেয়াদী চিকিৎসায় ৩৩ মি.গ্রা./ কেজি. (দৈহিক ওজন) এ মাত্রা হ্রাস করা যায়। অথবা চিকিৎসকের পরামর্শ অনুযায়ী সেব্য।

Should be taken with food.

পার্শ্বপ্রতিক্রিয়া

Nalidixic Acid Chin Tien সাধারণত সুসহনীয়। ক্ষেত্রবিশেষে গ্যাস্ট্রোইন্টেস্টাইনাল অতি সংবেদনশীলতা, জ্বর, ইউসােনােফিলিয়া, মৃদু আলােক সংবেদনশীলতা ও নিউরােলজিক্যাল প্রতিবন্ধকতা যেমন ঝিমুনি, অবসন্নতা পরিলক্ষিত হয়।

সতর্কতা

• নাইট্রোফিউরান্টয়েন ও এন্টি-কোয়াগুলেন্টের সাথে Nalidixic Acid Chin Tien সেবন করা উচিত নয়।
• কিডনীর অক্ষমতায় ইহার প্লাজমা হাফ-লাইফ ৮ ঘন্টা হতে ২১ ঘন্টায় বৃদ্ধি পেতে পারে।
• Nalidixic Acid Chin Tienের মাত্রাধিক সেবন শিশুদের ক্ষেত্রে ইন্ট্রাক্রেনিয়াল চাপ সৃষ্টি করতে পারে, তাই ইহা শিশুদের ক্ষেত্রে সতর্কতার সাথে ব্যবহার করা উচিত।
• রেজিস্ট্যান্ট স্ট্রেইন ডেভেলপমেন্ট এড়ানাের জন্য দীর্ঘমেয়াদী শিগেলা সংক্রমণ চিকিৎসায় ইহা সংক্রমণের ৪৮-৭২ ঘন্টা পর ব্যবহার করা উচিত।
• তিন মাসের কম বয়স্ক শিশুদের ক্ষেত্রে Nalidixic Acid Chin Tien নির্দেশিত নয়।
• মৃগী রােগীদের ক্ষেত্রে ঔষধটি প্রতিনির্দেশিত।
• Nalidixic Acid Chin Tien অতিসংবেদনশীল রােগীদের ক্ষেত্রেও প্রতিনির্দেশিত।
• যকৃৎ ও কিডনী জটিলতায় ইহা প্রতিনির্দেশিত।

মিথস্ক্রিয়া

Concomitant use of Nalidixic acid with melphalan there have been reports of death froms severe blood containing diarrhoea caused by hemorrhagic ulcerative colitis. Probenecids inhibits tubular secretion of nalidixic acid and may therefore elevate serum concentration, possibly enhancing toxicity. Chlorpromazine and Perphenazine have been shown to potentiate the effect of Nalidixic Acid in vitro.

গর্ভাবস্থাকালীন ব্যবহার

Pregnancy: There is the possibility that it may cause cartilage damage and as it is a DNA-gyrase inhibitor there is a possibility of causing DNA damage too. Nalidixic acid is excreted in breast milk and there is a report of hemolytic anaemia in a breast feed child of an azotemic mother.

বৈপরীত্য

Nalidixic Acid is contraindicated to known hypersensitivity to Nalidixic acid, history of convulsive disorders and patients with porphyria. Should be used with caution in patients with liver and renal disease. Also in glucose 6 - phosphate dehydrogenase deficiency.

Renal Impairment: Reduced doses should be considered.

Hepatic Impairment: Reduced doses should be considered.

তীব্র ওভারডোজ

Symptoms: Toxic psychosis, convulsions, increased intracranial pressure, metabolic acidosis, vomiting, nausea, lethargy.

Management: Increase fluid admin; supportive measures. Anticonvulsants may be used in severe cases.
