ইভারমেকটিন বেছে বেছে গ্লুটামেট-গেটেড ক্লোরাইড আয়ন চ্যানেলগুলির সাথে বেঁধে থাকে এবং উচ্চ স্নেহের সাথে থাকে, যা ইনভারটাইবারেট স্নায়ু এবং পেশী কোষে ঘটে স্নায়ু বা পেশী কোষের হাইপারপোলারাইজেশন সহ ক্লোরাইড আয়নগুলির কোষের ঝিল্লির ব্যাপ্তি বৃদ্ধি পায় পরিণামে পরজীবীর মৃত্যু হয় ।

Sklice মূলত পরজীবীর নার্ভ ও অন্যান্য কোষে আক্রমণ করে একে পরাস্ত করে।

Ivermectin is indicated for the treatment of the following infections:

Strongyloidiasis of the intestinal tract.Ivermectin is indicated for the treatment of intestinal (i.e., nondisseminated) strongyloidiasis due to the nematode parasiteStrongyloides stercoralis.

This indication is based on clinical studies of both comparative and open-label designs, in which 64-100% of infected patients were cured following a single 200-mcg/kg dose of ivermectin.

Onchocerciasis: Ivermectin is indicated for the treatment of onchocerciasis due to the nematode parasite Onchocerca volvulus.

This indication is based on randomized, double-blind, placebo-controlled and comparative studies conducted in 1427 patients in onchocerciasis-endemic areas of West Africa. The comparative studies used diethylcarbamazine citrate (DEC-C).

NOTE:Ivermectin has no activity against adultOnchocerca volvulusparasites. The adult parasites reside in subcutaneous nodules which are infrequently palpable. Surgical excision of these nodules (nodulectomy) may be considered in the management of patients with onchocerciasis, since this procedure will eliminate the microfilariae-producing adult parasites.

Oral

Filariasis:

• Adult: Dosing regimen depends on the causative agent. Mansonella streptocerca 150 mcg/kg as a single dose; Mansonella ozzardi 200 mcg/kg as a single dose.
• Child: ≥15 kg: Dosing regimen depends on the causative agent. Mansonella streptocerca 150 mcg/kg as a single dose; Mansonella ozzardi 200 mcg/kg as a single dose.

Scabies:

• Adult: Sarcoptes scabiei 200 mcg/kg as a single dose, repeat dose in 2 wk.
• Child: ≥15 kg: Sarcoptes scabiei 200 mcg/kg as a single dose, repeat dose in 2 wk.

Onchocerciasis:

• Adult: 150 mcg/kg as a single dose; retreatment may be given every 6-12 mth until adult worms die.
• Child: >5 yr and ≥15kg: 150 mcg/kg as a single dose every 6-12 mth until adult worms die.

Ascariasis:

• Adult: Ascaris lumbricoides 150-200 mcg/kg as a single dose.
• Child: ≥15 kg: Ascaris lumbricoides 150-200 mcg/kg as a single dose.

Strongyloidiasis:

• Adult: 200 mcg/kg as a single dose for 1-2 days.
• Child: >15 kg: 200 mcg/kg as a single dose for 1-2 days.

Gnathostomiasis:

• Adult: Gnathostoma spinigerum: 200 mcg/kg once daily for 2 days.
• Child: ≥15 kg: Gnathostoma spinigerum: 200 mcg/kg once daily for 2 days.

খালি পেটে খাওয়া উচিত।

পার্শ্বপ্রতিক্রিয়া

ডায়রিয়া, বমি বমি ভাব, বমি, মাথা ঘোরা, প্রুরিটাস, মূত্রাশয়, ফুসকুড়ি, আর্থ্রালজিয়া, জ্বর, মায়ালজিয়া, অস্থিরিয়া, পাকস্থলীর হাইপোটেনশন, টাকাইকার্ডিয়া, শোথ, লিম্ফডেনোপ্যাথি, কাশি, মাথাব্যথা, অসময়ে, ক্ষণস্থায়ী ইওসিনোফিলিয়া, লিভারের এনজাইমের মান উত্থাপন।

সতর্কতা

Concurrent Loa loa infection, impaired blood-brain barrier function due to infection.

মিথস্ক্রিয়া

অ্যালকোহল এর দ্বারা লেভামিসোল জৈব উপলভ্যতা বৃদ্ধি পেতে পারে।

গর্ভাবস্থাকালীন ব্যবহার

বিভাগ সি: পশুপাখির অধ্যয়ন ভ্রূণের উপর বিরূপ প্রভাব প্রকাশ করেছে (টেরেটোজেনিক বা ভ্রূণক্ষেত্র বা অন্যান্য) এবং মহিলা এবং প্রাণীতে নিয়ন্ত্রিত অধ্যয়ন পাওয়া যায় নি। তাই এ ওষুধ তখনই দেওয়া উচিত যদি সম্ভাব্য সুবিধা ভ্রূণের সম্ভাব্য ঝুঁকিটিকে ন্যায়সঙ্গত করে তোলে।

বৈপরীত্য

Hypersensitivity. Pregnancy and lactation. Children <15 kg body weight.
